Business Finance forms
Westpac standard forms should be used by Brokers when applying for our products. If you'd like to use your own application template, it must be submitted to us for approval before use. You will also need to attach a Privacy Consent and Acknowledgement form, signed by the applicant.
ID Request Form: Individual Equipment Finance and Business Broker
Use this form to apply for a Westpac individual Business or Equipment Finance Broker identification number. An ID number is required to submit deals and obtain commissions.
ID Request Form: Individual Referrer
Use this form to apply for a Westpac individual Referrer identification number. An ID number is required to submit and track deals, and obtain commissions.

Business Finance Introducer Application
Use this form for all Business Finance applications, except when the loan is for Invoice Finance only. In this case, use the Business Finance and Equipment Finance Applicant Referral form.

Serviceability Worksheet
This is used to determine the capacity of the applicant to repay the loan facility. If the applicant's serviceability does not meet our requirements, they may not be approved. Use the guide to help you complete the worksheet. A completed serviceability worksheet must accompany every application.

Working Assets Statement
If finance is being secured by business assets, this form must be completed to enable an assessment of the value of tangible business assets, which may include stock, debtors, plant equipment and motor vehicles.

Business Finance and Equipment Finance Applicant Referral
If you are an accredited referrer, use this for all referrals. If you are an accredited broker, but have an application for Invoice Finance only, complete this form instead of the Business Finance Introducer Application form.

Privacy Consent and Acknowledgement Form
This form must be signed by the applicant and submitted with the loan application if you are using your own application template.
