Westpac standard forms should be used by Brokers when applying for our products. If you'd like to use your own application template, it must be submitted to us for approval before use. You will also need to attach a Privacy Consent and Acknowledgement form, signed by the applicant.
ID Request Form: Individual Mortgage Broker
Use this form to apply for a Westpac individual Mortgage Broker identification number. An ID number is required to submit deals and obtain commissions.
